NHK: Japanese Government Report "It says the cause of the accident has not been fully disclosed"

Majia here: I'm confused because I thought the government report was released last week but NHK is claiming this is "breaking news."

NHK "Breaking News": "Government Panel Releases Fukushima Report"

[excerpted] "A government panel investigating the nuclear accident at the Fukushima Daiichi plant says the operator of the plant lacked a sense of crisis and imagination for possible tsunami.

It says Tokyo Electric Power Company should realize that Japan is prone to natural disasters and change its attitude toward disaster-preparedness....

It says the cause of the accident has not been fully disclosed...."

Majia here: Again, I don't know if this is yet another new report, distinct from the Diet report that came out a couple of weeks ago:

Japan National Diet “The Official Report of the Fukushima Nuclear Accident Independent Investigation Commission (2012): Published by The National Diet of Japan The Fukushima Nuclear Accident Independent Investigation Commission.
